this is an extremely good questionnaire! Thanks for providing it.

In my surveys I have other closing questions which focus on success.

What are you proud off?
What was the single most frustrating part?
How would you do things differently next time to avoid this frustration?
If you could wave a magic wand and change anything about the project, what would you change?
What made you not sleeping at nights?
What was the one thing you would exactly do again?
What was good but should be institutionalized or improved nevertheless?


And naturally I adapt the questions concerning section 3 according to the project type and SWLC model the company is using.
